The child's Original Birth Certificate with registration number
and any 3 of the following with the parent/guardian name-
Current Telephone Bill - (Not cell phones)
Medical or Car insurance policy
Mortgage Documents
Current bank statement
Driver's License or State ID
For Students entering Chicago Public Schools for the first time:
Current Physical (Medical) report
Information about child's progress and grade level at their last school.
